Taking the first step...
Thank you for considering The Sanctuary at The Lake. For many, admitting that there is a problem and seeking help is the most difficult hurdle. Our staff is here to help you and your family members every step of the way. We invite you to contact our higly trained admissions team to answer any questions you may have. They will also be able to help you begin the assessment process. Our primary goal is for you to feel welcome and safe.
Our Intake Coordinator will ask you for information such as:
- Current symptoms necessitating inpatient treatment
- Current medical problems for which client is undergoing treatment
- Current medication usage
- Insurance information (social security number, insurance carrier, applicable group and member numbers)
- Home address and phone number
- Names, addresses, and phone numbers of treatment providers for both emotional and physical needs
- Transportation plans to and from the hospital
Our Intake Coordinator will also speak to you regarding our safety expectations and provide the client with information as to what to bring and what not to bring to the hospital. Some of our basic guidelines will be explained (smoking policy, cell phone, responsibility levels, visiting hours).
